Managing cerebrospinal fluid remains a challenge across neurological care, requiring significant clinical resources while impacting patient access and surgical precision. Two Cleveland Clinic Innovations technologies developed by clinician-inventors Sean Nagel, MD, Staff, Neurological Institute, and Andre Machado, MD, PhD, Institute Chair, Neurological Institute, are rethinking these long-standing challenges: Ambulatory Cerebrospinal Fluid (CSF) Management Device – Invented by Dr. Sean Nagel, this wearable system is designed to automate CSF drainage during normal pressure hydrocephalus (NPH) evaluations, reducing the need for intensive bedside monitoring while allowing patients to remain ambulatory. Anti-Brain Shift Device – Invented by Dr. Andre Machado, this novel burr hole device helps minimize brain shift during stereotactic neurosurgery by maintaining fluid at the surgical opening, supporting more precise targeting of delicate brain structures. Both technologies were developed to address meaningful unmet needs in neurosurgical care and have the potential to improve workflow efficiency while enhancing patient care. Cleveland Clinic is advancing a promising new targeted protein degrader platform with the potential to transform treatment for serious neurological disorders. Developed by Yu-Shang Lee, PhD and Ching-Yi Lin, PhD, these novel peptide-based, lysosome-mediated protein degraders are designed to specifically bind disease-driving proteins and direct them to the lysosome for elimination. The lead candidate targets chondroitin sulfate proteoglycan, a major component of glial scarring that prevents recovery from spinal cord injury. It leverages membrane permeability, target-specific binding, and lysosome-targeting functionality in a single peptide construct, enabling efficient delivery and degradation of the target. Beyond spinal cord injury, the platform is being expanded to additional indications, including chronic pain, peripheral nerve injury, ischemic stroke, amyotrophic lateral sclerosis, and Alzheimer’s disease, creating a pipeline opportunity in areas of unmet need. The program has demonstrated therapeutic efficacy and validated mechanism of action in preclinical models. “What does good surgery look like? What does standardization of care look like? What is it that makes outcomes differ — between surgeons, techniques, approaches?” These are the questions clinician inventor Cara R. King, DO, MS, Women’s Health, is asking as she thinks about innovation as it relates to artificial intelligence. She believes that incorporating AI into the surgical suite will provide a new kind of lens into what makes surgery successful. “We've been honestly very narrow in the way that we've measured outcomes,” she says. “We typically measure outcomes after the fact. What surgical AI is allowing us to do is look upstream — actually within the case itself.” That upstream view, she says, can offer a world of new insight. "Data isn't going to tell us where the gap is. It's going to make the invisible visible. Surgical AI will be the signal for us to know where to focus our energy to enhance performance."
