For a year, every night incident at one US 3PL was closed by the CTO personally. Phone call, laptop, 45 to 90 minutes to respond. 35,000 shipments a day hanging on one person's sleep. We measured it: a four hour failure in the peak window costs $80,000 to $120,000 in SLA penalties. The fix was not heroics. SLOs on every critical integration, runbooks, tiered on-call across three time zones. MTTR dropped to 11 minutes. Zero unresolved P1s in 12 months. The CTO went back to strategic work.
