Yesterday I was at the Hubert H. Humphrey Building for the one-year anniversary of the CMS Health Technology Ecosystem, representing Trial Library. Three observations worth sharing: 1) The numbers are real. Patient record access increased from 5% to 60% over 12 months (target: 80% by October). TEFCA moved 1.4B records in the last year vs. 10M in its first two. This isn't a pilot anymore; it's infrastructure. 2) The gap has moved. Patients are ahead of providers - they're showing up with QR codes carrying full FHIR records, and clinics don't know what to do with them. Year Two is all about provider adoption. 3) Trial matching was named from the podium. HHS called out clinical trial matching as a barrier being removed between patients and better care. This is why interoperability matters to Trial Library: with complete records at the point of care, surfacing every trial a patient qualifies for becomes a workflow, not a research project - especially in community oncology, where 85% of patients receive care. #CMS is not short on theatrics; the clipboard got a funeral yesterday. The next chapter is making sure what replaces it reaches every patient - not just the ones at flagship academic centers. Last week, we had the privilege of welcoming Chris Carr, co-founder of Black Land Ownership, to our company all hands. We started by asking him a simple question: What drives you? His answer had nothing to do with accolades or metrics. He talked about art. About living life with intention. About choosing optimism as a practice, not a default. What followed was one of the most honest conversations our team has had - about health equity, about trust, about what it actually means to serve communities that have been failed by institutions for generations. One line stayed with everyone in the room: "Don't use compliance as a ceiling. Do more than the bare minimum" Chris was diagnosed with acral melanoma in 2019. He navigated a system that misdiagnosed him twice. When he started asking why there was so little data on patients who looked like him, the answer he kept getting was: figure it out yourself. He didn't just survive it. He built something from it. At Trial Library, we exist because clinical trial access is not distributed equally. We think about this every day — in the tools we build, the communities we work in, and the patients we navigate. But Chris reminded us that knowing the gap exists and closing it at the level it deserves are two very different things. Conversations like this one remind us why that work matters — and how much further we have to go.
