Starting in 2027, the AMA is eliminating global obstetric billing — the longstanding practice of charging one flat fee to cover an entire pregnancy: prenatal visits, labor, delivery, and the postpartum checkup, all bundled into a single payment reconciled after the pregnancy ends. Under the new model, every individual service gets its own billing code and claim. Prenatal visits, labs, screenings, telehealth consultations, and postpartum care will each be coded, submitted, and reimbursed separately, as they happen. That's a real shift in how maternity claims will flow through a health plan, and most employers haven't heard about it yet. My team at Reproductive & Maternal Health Compass (RMH Compass) has been following a huge shift coming to maternity care billing — one that's approaching fast, and one that most experts aren't talking about yet: maternal health unbundling. For decades, obstetric care has run on global billing: a single flat fee that's covered pregnancy care from the first prenatal visit through the postpartum checkup. This is all about to change: starting January 1, 2027, the global billing codes will be replaced with fee-for-service billing — every prenatal visit, lab, screening, and postpartum service will be billed and reimbursed separately, as it happens. This shift is cause for real concern. Costs are set to rise for employers and patients alike, and it could make maternity care harder to access — putting pregnancy health outcomes at risk.
