In today’s healthcare environment, delivering high-quality care requires more than innovation. It requires care models that work in real-world settings — for patients, providers, and payers alike. At OneHome, our value-based approach is designed to help post-acute patients receive the right level of care, in the right setting, at the right time. But we know that successful care delivery depends on more than processes and protocols. It depends on strong relationships, clinical expertise, and empathy for the day-to-day realities providers face. Proud to spotlight the clinician-leaders behind this work and their role in building stronger partnerships across the post-acute care ecosystem. About us
Enabling value-based, post-acute care. As post-acute care strives for better health and financial outcomes benefiting patients and plans, providers must have the capability to implement comprehensive care. Our model creates true alignment between the health plan, delegated at-risk entity, clinicians in the field and members, leading to better care, higher satisfaction and lower costs. Our management model enables a broad range of post-acute care options including home healthcare, DME, home infusion, SNF and gap-closure services through our robust value-based model. We go beyond the typical utilization and network management models, leveraging high-performing providers to help ensure patients receive the right amount of care in the most appropriate setting. How we deliver care inside and outside the home is as vital to our model as what we provide. Our OneHome infrastructure enables us to close gaps, deliver better clinical outcomes and control costs with home healthcare agencies, infusion pharmacies, DME operations, SNF networks and many other areas of post-acute care. OneHome integrates all the tools available to deliver value directly to the patient and to deliver savings to the provider.

As demand for in-home recovery grows and DME spending rises, traditional fee-for-service models can contribute to overutilization, fragmented supplier networks and inconsistent service standards. By aligning incentives around timely, appropriate equipment use, there are models to help reduce inefficiencies, simplify referrals for physicians, support smoother hospital-to-home transitions and improve outcomes for patients, payers and providers. Glad to see Arun Dahiya share his thoughts at “Site-of-Care Showdown: How MA, Medicaid and Value-Based Models Are Reshaping Where Patients Go Next” during the HHCN Payer Summit. As the post-acute care landscape continues to evolve, aligning payers and providers around better member outcomes has never been more important. Arun discussed how value-based models are reshaping post-acute care, including the importance of meeting providers where they are on their journey toward risk-based arrangements. Proud to see this important conversation on the future of site-of-care decisions and value-based models included as part of the HHCN Payer Summit! #HHCNPayerSummit #PostAcuteCare

This week, Cassie Houff, MBA and Noel Gilliam, CLTC joined industry peers for an engaging fireside chat on post-acute care challenges and opportunities at Medical Cost Containment Series (HPRI) in Dallas. At OneHome, we currently serve 8.5M lives nationally, and our goal is to drive better outcomes by aligning incentives across payers and providers. True value-based care models require collaboration, evidence-driven strategies and flexible models tailored to each patient population. By focusing on transparency, shared accountability and clinically driven placement decisions, we can expand access and improve recovery journeys. OneHome is grateful to have been part of this conference to share best practices and explore ways to further scale these models together! #MCC
