Health systems don’t need just post-discharge call. They need fewer readmissions. Reducing readmissions means staying with a patient through the entire transition home: checking symptoms, answering medication questions, engaging caregivers, coordinating follow-up and recognizing when someone needs clinical help. That takes more than one interaction. And more than one agent. Our AI Readmission Orchestrator brings together specialized AI agents across the post-discharge journey, with a supervising brain determining what each patient needs next. The goal is not to automate one more piece of the workflow. It’s to give health systems the capacity to engage every discharged patient, across the full 30 days, with the intensity and personalization needed to actually move readmissions.

NEWS: Healthcare AI is moving beyond the single agent. Today, Hippocratic AI is introducing Agentic Orchestrators: coordinated teams of conversational AI agents designed to deliver outcomes, not tasks. After 250M+ patient interactions, one thing became clear: solving big problems like reducing readmissions, improving Star Ratings, managing chronic disease or bringing patients back into care won't be addressed with one call or one agent. It takes a team—and intelligence coordinating that team. Our orchestrators determine which agent engages which patient, when, and how, adapting to each individual while operating across entire populations. We’re launching 30+ orchestrators across providers, payers, pharma and med tech, from AI Readmission and AI Chronic Care Management to AI STAR Rating Improvement, AI Lost to Follow Up and AI Trial Enrollment. All powered by our safety-validated Polaris architecture. One agent completes a task. An orchestrated team can deliver an outcome. Welcome to the next generation of healthcare AI. Our CTO William Hudson, MBA knows a thing or two about healthcare gaps…and how AI steps in.
Lost-to-care is often multiple issues all coming together with a single result: a patient doesn't get the care they need from the system they trust. Maybe the patient never called, or called too late, or worse, booked, then was left in silence waiting for an appointment weeks out. Here are my thoughts on how CIOs can partner to expose the leak, and how AI finally makes fixing it scalable. #HealthcareIT #HealthcareAI #DigitalHealth #Healthcare #HippocraticAI

When a patient coughs mid-sentence, a clinician notices. Traditionally AI doesn’t. It filters the cough out as background noise and continues on. That is a health issue and a safety issue. Our research team built HealthCUES to change that. 🔊 Play the video to see how. The research has been accepted at SIGdial 2026, where it will be presented this week, August 2–5. And it’s already being integrated into Polaris, Hippocratic AI’s patented constellation model, helping patients in real time. HealthCUES is the first system to monitor respiratory sounds inside a live, real-time conversation. It doesn't just detect a cough, it distinguishes coughing from throat clearing, classifies the type (dry, wet, barking, whooping), and measures duration, all in about a third of a second, without interrupting the conversation. Why it matters: patients often don't mention a cough. They've normalized it; or don't think it's relevant. A clinician hears it anyway and asks the follow-up questions. An AI agent that only reads the transcript misses that signal entirely. HealthCUES restores it: the agent can notice a wet cough the patient never mentioned, and bring it up in conversation, catching potential serious health issues or flagging the pattern for human review. The results: 93% F1 on cough detection across 847 conversational audio segments, with licensed healthcare professionals confirming the clinical value of the subtype information in telehealth workflows. This isn't diagnosis. It's something more basic, teaching AI to pay attention to the patient, not just the transcript. Congratulations to the team, Tanmay Laud, Herprit Mahal MD, FACP. Subhabrata (Subho) Mukherjee. This is a big step forward for how Hippocratic AI is helping healthcare companies deploy the safest, most clinically complete conversational agents. Two years ago, WellSpan Health became one of the first major health systems in the world to launch our generative AI healthcare agents — starting with a single use case helping patients access life-saving cancer screenings. Today, that innovative partner is taking the next step. We're proud to announce an expanded multi-year partnership with WellSpan that moves from individual use cases to a platform-wide commitment, extending AI-powered capacity across inbound, outbound, ambulatory, and inpatient settings. Ana, WellSpan's AI agent built with Hippocratic AI, now manages more than 160,000 patient calls every month. That's over 7,000 hours of conversation. Next: post-discharge follow-up, chronic disease check-ins, and proactive outreach to close gaps in care. We're also establishing a dedicated on-site team of clinical, engineering, and deployment specialists at WellSpan's York, PA campus, because transformation happens shoulder to shoulder, not at arm's length. Thank you to Roxanna Gapstur Ph.D., R.N., President and CEO, Mark Kandrysawtz, MBA, Chief Innovation Office and the entire WellSpan team for continuing to lead the way. This is what it looks like when a health system doesn't just pilot AI, it scales it. https://lnkd.in/ejFCkssA
