Check out my new article, a collaboration with Dr. Marc Harrison. I’ve been thinking about what it will really take to fundamentally redesign American healthcare. It’s a question that runs through my new book, The Optimistic Leader’s Guide to Healthcare (ACHE Learn, June 2026). It’s also at the heart of an article I coauthored with Dr. Marc Harrison, a physician executive and global healthcare leader who has run major health systems and walked away from the traditional model to test better ones. Titled “Healthcare Isn’t Broken. It’s Doing What It’s Designed to Do,” it’s live now on the American College of Healthcare Executives blog. This is a conversation the industry needs to have, and ACHE’s platform puts it in front of the leaders who can do something about it. I’m grateful for Marc’s collaboration and for the opportunity to think through these questions together. This is just the beginning. $250 million a year 13% of the entire healthcare budget That's what Bank of America spends on GLP-1s for 211,000 employees. Brian Moynihan said it out loud on CNBC yesterday Two reactions I've heard today: "That's unsustainable" "That's the cost of doing right by your people" Both are wrong, or at least incomplete. The number by itself tells you nothing. What matters is what's inside it How many of those members are clinically appropriate? How many are on the right dose? How many have been on therapy for two years with no plan for what comes next? How many should have stepped down six months ago? Moynihan is negotiating hard on price. Smart. But price is the part a PBM can take back next year If you can't answer the questions above about your own population, you don't have a cost problem. You have a visibility problem What's your number?

📺 This morning’s Good Morning America cover story asked an important question: Can GLP-1 medications boost fertility? GLP-1s are not fertility medications. But for some women with obesity, insulin resistance and/or PMOS, treating underlying metabolic dysfunction can improve ovulation and fertility - sometimes before they realize it. Another critical point: tirzepatide, sold as Mounjaro and Zepbound, may temporarily reduce the effectiveness of oral birth control. The FDA recommends backup or non-oral contraception for four weeks after starting treatment and for four weeks after every dose increase. These medications are powerful tools, but every prescription should include a conversation about reproductive goals, contraception and pregnancy planning.
