Non–clear cell RCC is not one disease. This new IMDC real-world analysis is a strong reminder that our first-line choices should be histology-driven and biology-driven, not one size fits all. A few takeaways that impact how I counsel patients: ➡️ Contemporary 1L strategies (IO-based combinations or cabozantinib) clearly outperform older VEGF TKI and mTOR inhibitors across non–clear cell RCC. ➡️ In papillary RCC, IO-IO, IO-VEGF, and cabozantinib show similar overall survival. That shifts the decision toward patient goals, comorbidities, and toxicity tradeoffs rather than chasing a single “best” regimen. ➡️ The clearest signal remains sarcomatoid differentiation. Even within non–clear cell histologies, IO-IO stands out with higher response and longer survival, consistent with what many of us see clinically. Real-world datasets won't replace randomized trials, but in rare and heterogeneous diseases they improve how we set expectations and make day-to-day decisions. Congrats to the IMDC investigators and authors for delivering data clinicians can use. Building on SUNNIFORECAST, PAPMET, and KEYNOTE-B61, the next step is designing trials specifically for non–clear cell RCC: histology-defined cohorts, NGS-informed biologic stratification, and therapies matched to subtype dependencies (MET-driven papillary, FH-deficient, mTOR-pathway–altered chromophobe). We also need later-phase studies, sized realistically for a rare disease, that can still define first-line standards. If we get this right, we can move from extrapolation to evidence for our non–clear cell RCC patients. Full paper linked in the comments.
