We are proud to announce our Accountable Care Organizations’ (ACO) results for the 2024 performance year of the Centers for Medicare & Medicaid Services’ Innovation Center's (CMMI) Accountable Care Organization Realizing Equity, Access and Community Health (ACO REACH) model. In 2024, our eight REACH ACOs – comprised of 1,500 primary care physicians operating across 13 communities and responsible for the total cost and quality of care for 121,000 #Medicare patients – continued to achieve strong performance. In 2024, our REACH ACOs achieved: 🟢 $229 million in gross savings, including $54 million savings to the Medicare Trust Fund. 🔵 An average quality score of 96%, with 5 of 8 REACH ACOs attaining High Performers Pool status, achieving superior quality results across all four quality measures. agilon also had 5 ACOs among the top 20 Standard ACOs in gross savings rate. This achievement reflects the power of physician partnerships and full-risk, value-based care to create lasting impact in communities across the country. We are grateful to have participated in this year’s America's Physician Groups (APG) Spring Conference, where Ravi Sachdev, founder of agilon health, and Eric Becker, vice president of direct contracting and Medicare innovation model strategies, shared their perspectives on the evolving value-based care landscape. From navigating Medicare innovation models to advancing physician-led care, the conversations throughout the conference highlighted both the progress made and the work ahead. Thank you to APG for convening leaders across healthcare to exchange ideas and help shape the future of accountable care.
Forging more full-risk partnerships between #MedicareAdvantage plans and provider groups will require alignment of all stakeholders to achieve superior health outcomes with sustainable costs. At this week’s #APGSpringConference, #SachinJain of #ScanHealth called for leadership by medical groups to transform their practices away from conventional #feeforservice payment. Panelists including #DennisO’brien of United Healthcare and #RaviSachdev of Agilon said both #CMS and health plans can do more to put actionable health care data into provider’s hands quickly. #BrandonSim of #AstranaHealth said a standardized #DivisionofFinancialResponsibility document for use between plans and providers would also help clarify responsibilities and foster alignment. Forthcoming #CMMI models in MedicareAdvantage may also spur change.
