4M Analysis Made Practical https://lnkd.in/gjYSFZtc When a process fails, one of the fastest mistakes we can make is asking, “Who caused it?” A better question is: “What conditions allowed it to happen?” That is where 4M Analysis becomes useful. 👥 MAN / PEOPLE Look beyond “operator error.” Consider training, skills, workload, communication, staffing, experience, and whether the standard was actually understood and practical to follow. ⚙️ MACHINE Check equipment condition, tooling, settings, capability, maintenance, wear, lubrication, vibration, precision, and whether the machine can consistently meet the process requirement. 📦 MATERIAL Investigate specifications, supplier variation, contamination, storage, handling, availability, lot differences, WIP conditions, and whether the material actually meets requirements. 📋 METHOD Review the work sequence, standard work, procedures, parameters, inspection points, scheduling, handoffs, change controls, and whether the documented method reflects what really happens at the Gemba. 🎯 Start with a clearly defined problem. What happened? Where? When? How often? How large is the gap? 🐟 Use the 4Ms as branches of a Fishbone Diagram to generate possible causes. 🔍 Then test those causes. Observe the process, review data, compare good versus bad conditions, ask 5 Whys where appropriate, and separate evidence from assumptions. This distinction matters: 4M Analysis organizes your thinking. It does not automatically identify the root cause. 🛠 Corrective action should target a verified cause, not the most convenient explanation. 📈 And the job is not finished when the action is implemented. Monitor the process and confirm that the problem does not return. A simple rule for problem solving: If your root cause is “the operator,” keep investigating. Good systems make the right way easier to follow and abnormal conditions easier to detect. It fails when leadership treats improvement as a project instead of the way the company is managed. Leaders cannot improve a process they only understand through reports, dashboards and conference-room discussions. 🔍 Larry Culp brings another critical lesson: change the way the work gets done. But the better question is: “What happens if we don’t, and they stay?” 💡 Training is not simply an employee benefit. It is part of your operating system. 🏭 When people are not developed, organizations pay for it through recurring problems, dependence on a few experts, inconsistent processes, slower improvement, poor succession planning, and decisions pushed upward because employees lack the capability or confidence to solve problems themselves. 📈 Developing people changes that equation. 🧠 Teach structured problem solving. 🔄 Cross-train critical roles. 📊 Use skill matrices to expose capability gaps. 🎯 Build coaching into daily management. 🛠️ Give employees opportunities to apply what they learn. The objective is not simply to “send people to training.” The objective is to create more capable people who create more capable processes. Some trained employees may eventually leave. From Hourly Data to Action https://lnkd.in/epzuCas The shift finished 33 units behind plan. Useful information? Yes. Enough information to improve the process? Not even close. An effective Hour-by-Hour Board turns production data into a management cycle that repeats throughout the shift. 🎯 Plan Define what should be produced during the next hour. Without a clear expectation, there is no meaningful gap to manage. 📊 Actual Record what really happened. In this example, the shift planned 435 units and produced 402. 🔎 Gap Make the difference visible immediately. A -3, -5, or -12 is not just a number. It is a signal that something in the process prevented the expected result. ⚙️ Loss / Problem Capture the reason while it is fresh: machine jam, material delay, changeover difficulty, downtime, rejects, staffing, or another constraint. 🛠️ Countermeasure Ask the most important question: What are we doing about it now? Temporary containment may keep production moving, but temporary action should never quietly become the permanent solution. 👤 Owner Every meaningful problem needs somebody responsible for moving it forward. “The team is working on it” is rarely strong accountability. ⏰ Due time Action without timing easily becomes intention. A target time creates urgency and enables follow-up during the same shift. 🚦 Status Fixed. Temporary action. Escalated. On target. That distinction matters. A machine running again does not necessarily mean the underlying problem has been solved. The real power of an Hour-by-Hour Board is not the board itself. It is the operating rhythm it creates: 👀 See the gap early. 🧠 Understand what caused it. 🔧 Take action while it still matters. 🙋 Assign ownership. ⏱️ Follow up quickly. 🔄 Prevent recurrence. Fix the Gap, Not the Symptom https://lnkd.in/g-Pic7p Does it move? Should it? It may be one of the funniest engineering flowcharts ever made, but underneath the joke is a surprisingly useful lesson in problem solving. 🔎 The first question establishes the ACTUAL condition. 🎯 The second question establishes the EXPECTED condition. When actual = expected, there may be nothing to fix. When actual ≠ expected, you have a gap. And that gap is where real problem solving begins. But in Continuous Improvement, that should only be the beginning of the conversation. 🩹 A quick fix restores the condition. 🔍 Root cause analysis explains why the condition occurred. 🛠️ Corrective action removes the cause. ✅ Verification confirms the problem stays solved. 📚 Standardization helps prevent the same problem from returning somewhere else. This distinction matters. A machine running again does not necessarily mean the problem has been solved. A defect disappearing does not necessarily mean the cause has been eliminated. A workaround becoming permanent is often how today's small inconvenience becomes tomorrow's chronic loss. The better troubleshooting flowchart is: 👀 What is happening? 🎯 What should be happening? ↔️ What is the gap? 🛡️ Do we need immediate containment? 🔎 Why did the gap occur? 🛠️ What eliminates the cause? ✅ How will we verify effectiveness? 📖 What should we standardize or share? Simple questions create disciplined thinking. 👣 Make It Safe to Follow https://lnkd.in/e_eu9SpN Leadership becomes visible when the path is uncertain. 🦆 A flock needs to cross a frozen lake. Everyone can see the destination, but nobody knows whether the ice will hold. The leader does not stand safely on shore and shout, “Go!” 👣 She steps onto the ice first. 🔎 She tests the surface carefully. ⚠️ She learns where the risk is. ✅ She moves forward only when the next step appears safe. And something important happens. The others begin to follow. This is a useful lesson for anyone leading Continuous Improvement. 💡 When introducing a new process, don’t simply tell people that it will work. Test it with them. 🏭 When changing standard work, go to the Gemba and see how the change actually affects the job. 🧪 When the solution is uncertain, run a small experiment before demanding full-scale adoption. 🛡️ When people hesitate, ask whether they are resisting change or whether leadership has failed to make the next step safe enough to take. The best leaders reduce uncertainty before asking others to accept it. They demonstrate. They test. They learn. They adjust. Then they invite others forward. Leadership is not pointing toward the destination. Too often, organizations learn 5S, Kanban, Kaizen, TPM or SMED independently. The bigger opportunity comes from understanding how these methods work together. 🏭 5S establishes stable, visual workplace conditions so abnormalities become easier to see. 🔄 Kaizen turns improvement into an everyday habit rather than an occasional project. 📦 Kanban creates a demand-driven signal for production, movement or replenishment, helping control WIP and flow. ⏱️ Just-in-Time focuses the system on producing what is needed, when it is needed, in the quantity needed. 🛑 Jidoka builds quality into the process by detecting abnormalities, stopping when necessary and preventing defects from moving downstream. 🛡️ Poka-Yoke prevents mistakes or detects them before they become defects. ⚖️ Heijunka levels production volume and mix, reducing the instability created by uneven demand. 🚨 Andon makes problems visible and calls attention to conditions requiring support. 👀 Genchi Genbutsu reminds leaders and problem solvers to go to the actual place and understand the actual process before reaching conclusions. 🎯 Hoshin Kanri connects strategic priorities with measurable objectives and actions. ⏳ SMED attacks changeover losses, while TPM improves equipment effectiveness through shared ownership, planned maintenance and continuous improvement. 🔁 PDCA provides the learning cycle: Plan, Do, Check and Act. ♻️ Muda, Mura and Muri help us recognize waste, unevenness and overburden. 🗺️ Value Stream Mapping then connects the bigger picture, showing how material and information move through the entire value stream. The lesson is simple: do not ask, “Which Lean tool should we implement?” Ask, “What problem are we trying to solve, and which method helps us understand and improve the system?” Small changes matter. It is about learning to see a process differently. A strong Lean Six Sigma foundation starts with a simple question: What is preventing this process from consistently delivering value to the customer? 🟡 Lean teaches us to see flow, identify waste, simplify work, and focus on what the customer actually values. 📊 Six Sigma teaches us to understand variation, use data, and reduce the causes of inconsistent performance. 🔄 DMAIC brings the thinking together: Define the problem and scope. Measure to establish baseline performance. Analyze to identify root causes. Improve by testing and implementing solutions. Control the process so the gains are sustained. 🧹 5S develops workplace discipline through Sort, Set in Order, Shine, Standardize, and Sustain. Done properly, 5S is not simply housekeeping. It makes abnormalities easier to see and good work easier to perform. 🎯 Lean principles take the thinking further: specify value, map the value stream, create flow, establish pull, and continue pursuing perfection. 🔍 Yellow Belts also begin building familiarity with practical tools such as 5 Whys, value stream mapping, cause-and-effect diagrams, control charts, standard work, takt time, Kanban, PDCA, stakeholder analysis, and visual control. 💡 The real value of Yellow Belt knowledge is not the belt itself. Look Beyond Human Error https://lnkd.in/gdRKa2jH “Operator error” is one of the easiest conclusions in manufacturing. It is also one of the most dangerous. When something goes wrong, blaming the person closest to the problem may feel fast, simple, and inexpensive. But if the same type of error can happen again tomorrow, we probably haven't solved anything. 🔍 Instead of asking, “Who made the mistake?” ask, “What allowed the mistake to happen?” ⚙️ Was the standard clear, current, and accessible? 🧰 Did the operator have the right tools, materials, information, and training? 👀 Could the process make the wrong action look like the right one? ⏱️ Did production pressure, workload, fatigue, or interruptions increase the probability of error? 🛡️ Could mistake-proofing, visual controls, automation, or a better process design prevent recurrence? This is where Continuous Improvement changes the conversation. “Human error” should rarely be the end of a root cause analysis. It should be the beginning of another question: Why was the system dependent on someone performing perfectly every single time? A strong process does not assume perfect people. It creates conditions where the correct action is easier, abnormal conditions are visible, and mistakes are difficult to make or quickly detected. That is the difference between correcting a person and improving a process. The first may stop the conversation. Stop Adding Passengers https://lnkd.in/gYJ-7a7V A process stuck in the mud rarely needs more people sitting in the wagon. It needs someone to understand why the wagon is stuck. 🚜 When performance drops, many organizations add more meetings, more follow-ups, more approvals, and more people watching the same problem. It may feel like action, but it often adds weight instead of removing the constraint. 🐴 In Lean Six Sigma, the first question should not be, “Who is not working hard enough?” The better question is, “What is preventing flow?” The mud could be waiting time, unclear ownership, poor maintenance, excessive approvals, rework, missing materials, bad scheduling, weak standards, or a bottleneck that everyone has learned to work around. 🛞 Adding pressure to a constrained process does not create flow. It usually creates frustration, firefighting, and more meetings about why the wagon still has not moved. Great leaders do not just sit in the wagon asking for updates. They go to the process, listen to the people closest to the work, study the data, identify the real constraint, and help remove the mud. ⚙️ More supervision is not the same as more capability. More reporting is not the same as problem solving. More people in the wagon is not the same as more pulling power.
